Care at home & in your community

Goshen’s nine support options

Read the option you are interested in, then send us a request. We’ll discuss suitability, your location and preferred times, and confirm availability and funding before any support begins.

Community nursing

Registered nurses care for health needs related to your disability, at home so you avoid unnecessary hospital visits.

What’s included

  • Wound care, dressings and pressure-injury prevention
  • Medication support, administration and reviews
  • Catheter, bowel and continence care
  • Watching for health changes and acting early
  • Working alongside your GP, specialists and hospital team

Who it suits

People whose disability-related care needs go beyond what a support worker can safely deliver on their own.

Funding & good to know

Nursing care related to your disability is often considered under Core supports, and complex needs may need extra funding. The NDIA decides what is included in your plan, and the NDIS does not fund general health care that Medicare or the health system covers. Goshen must assess your clinical needs before we can confirm we can deliver the care safely.

How to apply:Send the request form below or call us — tell us your suburb and the days or times you need.

Personal care

Respectful, unhurried help with the personal tasks that start and finish your day.

What’s included

  • Showering, bathing, grooming and oral care
  • Dressing and getting ready for the day
  • Morning and evening routines, at your pace
  • Toileting and continence support
  • Skin checks and mobility help around the home

Who it suits

People who need hands-on help with personal tasks to stay safe, comfortable and ready for the day.

Funding & good to know

Personal care is usually considered under Core supports, in Assistance with Daily Life. The hours in your plan come from an assessment of what you need — Goshen never assumes a set number of hours.

How to apply:Send the request form below or call us — tell us your suburb and the days or times you need.

Supported Independent Living

Daily living support in a home that is truly yours, with staff rostered around your routine.

What’s included

  • Support through the day, including early mornings and overnights
  • Meals, household tasks, budgeting and routines
  • Help keeping your tenancy and good relations with neighbours
  • Support shared with others or provided to you alone
  • Workers chosen to match your personality and preferences

Who it suits

People who need daily assistance to live independently at home, including overnight or 24-hour support.

Funding & good to know

SIL is funded under Core supports and is assessed individually by the NDIA. It does not cover rent, groceries or ordinary living costs. Arrangements depend on current vacancies and who you would like to live with, so availability must be confirmed with Goshen.

How to apply:Send the request form below or call us — tell us your suburb and the days or times you need.

Community participation

More time out and about doing things you value, with people you enjoy being around.

What’s included

  • Outings, classes, sport, hobbies and events
  • Travel training and practising public transport
  • Social groups and building lasting friendships
  • Volunteering and local community activities
  • Support to feel confident in new places

Who it suits

People who want more social and community time, and support to get there safely.

Funding & good to know

This is usually considered under Core supports, in Assistance for Social and Community Participation, or under Capacity Building supports for social engagement. Your NDIA confirms what is funded in your plan.

How to apply:Send the request form below or call us — tell us your suburb and the days or times you need.

Transport & appointments

Practical help getting where you need to go, whether that is the hospital, the shops or work.

What’s included

  • Travel to medical, therapy and NDIA appointments
  • Support with shopping and errands
  • Travel training and public transport practice
  • Getting to work, study or volunteering
  • Help getting in and out of vehicles safely

Who it suits

People whose disability makes using transport on their own unsafe or impractical.

Funding & good to know

Transport support is usually considered under Core supports, in Assistance with Travel and Transport, while travel training may sit under Capacity Building. Transport funding is individual to your plan — we will look at what you have before quoting.

How to apply:Send the request form below or call us — tell us your suburb and the days or times you need.

Household tasks

Help keeping your home clean, comfortable and running the way you like it.

What’s included

  • Regular cleaning, laundry and linen changes
  • Meal preparation and planning shopping lists
  • Tidying, organising and home safety checks
  • Garden and yard help where it relates to your disability
  • Support that keeps your home ready for visitors

Who it suits

People who cannot safely manage household tasks themselves because of their disability.

Funding & good to know

Household help related to your disability is usually considered under Core supports, in Assistance with Daily Life. Tasks you could do yourself are not disability-related and cannot be funded by your plan.

How to apply:Send the request form below or call us — tell us your suburb and the days or times you need.

Structured day programs

Friendly, planned days where you build skills, make friends and get out and about.

What’s included

  • Weekly group or individual activities
  • Everyday skills, communication and confidence building
  • Creative, social and physical activities
  • Community access during the day
  • Regular catch-ups about your goals and progress

Who it suits

People who want a meaningful, structured day with staff support and other participants.

Funding & good to know

Day programs are usually considered under Core supports, in Assistance for Social and Community Participation, or under Capacity Building supports. Places depend on current availability in your area.

How to apply:Send the request form below or call us — tell us your suburb and the days or times you need.

Housing & respite

A break for you or your family, and practical help finding a home that suits your needs.

What’s included

  • Short stays and holiday breaks with staff support
  • Respite that gives family carers a rest
  • Help searching for, viewing and moving into a home
  • Support while you work out longer-term living options
  • Familiar staff so a stay feels like home

Who it suits

Participants who need a change of routine or a short-term safety net, and families planning ahead for where they or their loved one will live.

Funding & good to know

Short stays are usually considered under Core supports as Short Term Assessment and Support. Long-term accommodation is a separate NDIA process and Goshen cannot promise a home. Availability must always be confirmed with us — an enquiry does not guarantee a place.

How to apply:Send the request form below or call us — tell us your suburb and the days or times you need.

Work & volunteering

Support to find, start and keep a job or volunteer role you actually enjoy.

What’s included

  • Exploring work or volunteering interests
  • Resume writing and interview practice
  • On-the-job training and workplace support
  • Travel, routine and communication help at work
  • Support to stay in the role once you start

Who it suits

People who want to start, return to or keep work or volunteering, with help at their own pace.

Funding & good to know

Employment-related help is usually considered under Core supports or Capacity Building supports. Your NDIA or support coordinator confirms what is in your plan before we begin.

These are general guidance notes, not Goshen eligibility decisions. The NDIA determines access and funding; Goshen confirms whether we can safely deliver your requested support.

Choose a date & request support

Clear costs before you begin

What does support cost?

Goshen’s prices follow the NDIS pricing arrangements. Your cost depends on the support you need and when it is delivered. Contact us for a personalised quote; fixed Goshen rates are not published here.

Your support needs
The type of support, length and frequency of visits, and how support is delivered all help determine your quote.
Days, times & other charges
Rates can vary by time and day. Ask whether travel or other permitted charges apply, so you understand the full cost.
Your plan & funding
We’ll discuss your plan management and relevant funding with you. Ask Goshen to confirm which arrangements we can accept.

NDIS price limits are not a quote

An applicable NDIS price limit is the maximum allowed for that support, not an automatic Goshen rate. The NDIS pricing arrangements apply to NDIA-managed and plan-managed funding; self-managed funding has different pricing rules. Funding must still be used in line with your plan.

Before you agree to begin, discuss the rate, expected hours, any additional charges and the service agreement with Goshen. Sending an enquiry does not book or commit you to a service.

What to have ready

  • Your suburb and the support you want

So we can check we can deliver it in your area and tell you honestly if we can’t.

  • Preferred days and times

These are preferences only — Goshen confirms availability with you before anything is scheduled.

  • How your plan is managed

Self-managed, plan-managed or NDIA-managed. If you don’t have a plan yet, tell us and we’ll point you to the right next step.

  • What matters to you
